An assessment is a conversation about what’s going on, what support feels realistic, and what options fit your needs. You don’t need to have everything figured out to start.

If you’re a parent, spouse, or family member reaching out for someone else, you can begin the process. Our team will guide you on next steps and how to involve your loved one.

Walk-in assessments are available on a first come, first served basis during clinic hours. Appointments are also available for in-person or virtual visits by calling our outpatient clinic.

You don’t need a referral to get started.

Clinic Hours

Walk-in appointments are available on a first-come, first-served basis. Please be aware that you may experience a wait before your evaluation.

Monday – Thursday: 8am – 5pm
Walk-Ins Accepted Until 2:30pm

Friday: 8am – 3pm
Walk-Ins Accepted Until 1:30pm

What to Bring (If You Have It)

  • Photo ID
  • Insurance Card or Insurance Information (If Applicable)
  • A List of Current Medications

If you don’t have everything, you can still come in or call.

What Happens During an Assessment

Please plan for approximately 10 minutes to complete paperwork. The one-on-one evaluation itself takes about two hours to complete. Please be aware that you may experience a wait before your evaluation.

We’ll talk through what you’re experiencing, your goals, and what kind of support may fit best. If safety is a concern, we focus first on the safest next step.

A bio-psycho-social assessment is used to better understand your situation and make a diagnosis, along with a recommendation for the appropriate level of care.

Our team looks at the full picture, including medical, psychological, and social needs. From there, we create a personalized care plan and connect you to the right services. You’ll have support from your first conversation through ongoing care.

How We Determine the Right Level of Care

The ASAM Criteria is the nationally recognized standard for assessing and matching individuals to the right level of care for addiction and co-occurring conditions. ShareHouse uses this framework to complete a comprehensive, multidimensional assessment of each person’s medical, psychological, and social needs, then develops an individualized care plan.

This approach strengthens our continuum of care in Fargo and Grand Forks, allowing people to step up or step down levels of care as life changes, while remaining connected to the support they need.

Recovery isn’t always a straight line. Needs can change, such as stress, support systems, health, and stability can all shift over time. This is not a one-size-fits-all approach. We meet each individual where they are at, helping you start at the level of support that fits now and adjust as life changes—without starting over.

How People Move Between Levels

There’s no single path.

Some people begin with outpatient care. Others start in residential. Many step down into outpatient support after completing residential treatment.

The “right” path is the one that fits your needs and keeps you connected to support along the way.

Do I need an appointment or can I walk in?

Walk-ins only. View our clinic hours.

Can someone come with me?

Support persons are welcome to come to the assessment clinic, however assessments are done on an individual basis. Exceptions may be made for interpreters or other approved accommodations.

How long does the intake process take?

Can I do my assessment over the phone or online?

We do provide Telehealth evals for clients who reside more than an hour away from F/M area. Please contact us to schedule.
